Foreword

The 10YHP outlines a bold vision for the NHS, shaped by strong engagement and collaboration. Our new podcast series Providers Deliver: 10-year health plan trailblazers showcases hospital, mental health, community and ambulance trusts working in geographically diverse areas.  

Each case study exemplifies how trusts are taking forward new and creative ways of working to ensure patients receive the care they need, while also contributing to a more sustainable future for the NHS. 

Through this series, we highlight how these trailblazing trusts are helping to address key themes such as integrated, patient-centred care at neighbourhood level; innovative models for mental health support; community-based service delivery; digital workforce transformation; and collaborative approaches to improving population health and reducing health inequalities. 

The focus rests on three priorities: empowering patients with access and control over their health information, strengthening local NHS connections through increased neighbourhood working, and using technology to boost productivity and transform care delivery.  

At a time when the NHS faces immense pressure, it is more important than ever to celebrate the progress being made and this podcast series does just that. We highlight the great work trusts are already doing to deliver the ambitions of the 10YHP.  

Trust leaders understand and accept the scale of the challenge and are already working to deliver meaningful change – these efforts deserve recognition.
